Our approachSupport doesn't end when the program does.
Our Aftercare program provides personalized, home-based support to help families achieve their goals. Each client collaborates with their case manager to develop an Individualized Service Plan and is supported through case management, goal planning, developmental screenings, and connections to community resources.
Clients may enroll in Aftercare within six months of discharging from a residential program and can receive services for up to three years. Meetings are typically held monthly. Aftercare is available for clients with or without children.
