Every family's goals are different. Our Aftercare Program provides personalized support focused on helping clients maintain stability, strengthen parenting and life skills, and continue moving toward long-term independence.

Our approach

Support doesn't end when the program does.

Our Aftercare program provides personalized, home-based support to help families achieve their goals. Each client collaborates with their case manager to develop an Individualized Service Plan and is supported through case management, goal planning, developmental screenings, and connections to community resources.

Clients may enroll in Aftercare within six months of discharging from a residential program and can receive services for up to three years. Meetings are typically held monthly. Aftercare is available for clients with or without children.

  • The Aftercare Program is available to clients living in the greater Charlotte area who have completed one of Crittenton's residential programs within the past three years.

  • Eligible families may receive continued support for up to three years after completing their residential program, depending on their individual needs and goals.

  • Home visits are tailored to each family's needs and may include parenting support, child development guidance, goal planning, education or employment assistance, healthcare coordination, and connections to community resources.

  • Yes. Our team helps connect families with housing resources, furniture assistance, healthcare providers, educational opportunities, employment resources, and other community services that support long-term stability.
